Use of Collection Control Forms

Because of the variety of ways in which collections arrive at NY to be processed, it is not likely that any single person will be aware of all of them.  To make possible the reporting of determinations to the proper person(s), the distribution of exchange to the proper herbaria, and the accurate transfer of miscellaneous collections processing information to future herbarium assistants, a Collection Control Form should be completed for each set of collections processed at NY for identification.  This applies to staff collections and, especially, to those made by other collectors but somehow processed here.

The collector (or curatorial assistant) should complete a Collections Control Form (see below) and send it to Amy Weiss.

Forms

Please complete this form for each set of collections processed for determination at NY -- your own or those of another collector you brought here.  If on one trip you collected in more than one country or on more than one project, and if different rules apply, complete a separate form for each series. If host institution is sending sets to specialists, please attach a list of specialists used.

The fillable version of the form can be completed on your computer and emailed to Amy Weiss.

If you enjoy the sensation of putting pen or pencil to paper, the original print-out version can also be used and given to Amy.
